Our teams are here to support every person in the way they want to be supported in their own environment. Our staff team is trained in Active Support and Reach Standards, which means they will give the support required but always ensuring the individual leads the activity which is completed at the pace that person feels comfortable with.
Typical Support Package
Stage 1 - Moving in Phase
This usually happens within the first week if we are giving support to find accommodation.
We have an initial support plan based on safety and immediate needs which will be implemented and also give support with tenancy and benefits, and help with personalising the living environment. Assistance is given with registering with primary care health services such as local GP, optician, dietician and dentist.
Stage 2 - usually in the first month
Once things start to settle, we revisit and reimplement support plans, as well as putting in place risk assessments, community safety assessments and an Individual Skills for Life Planner. We will design activities to suit individual needs, always with guidance from Community Mental Health Teams and / or Community Learning Disability Teams, or our own multidisciplinary team if individual needs require it.
Stage 3 - needs dependent
We ensure the best support is given to meet physical and mental health needs. This includes help with fully integrating within the community through finding placements for education, training or employment.
